Diagnostic Assessment


Diagnostic Assessment services are appropriate for individuals , ages 12 years through adulthood, who
  • Suspect they may have autism spectrum disorder (ASD) and want to find out if they do.
  • Are concerned they may have ASD and want to find out if they do.
  • Not sure they have ASD and want to find out if they do.
  • Have ASD and want an update on their symptoms and to get recommendations for supports and treatment. (Not all services are billed to insurance.)
  • Have ASD and are concerned they may have a mental health condition such as anxiety and depression.
  • Have ASD and a mental health condition such as anxiety and depression and want treatment recommendations. (Not all services are billed to insurance.)
Diagnostic assessment includes a review of records, interview and direct assessment with the patient, interview with one or more caregivers, completion of rating scales by one or more caregivers, and a report that summarizes assessment findings and provides a diagnosis and general recommendations related to diagnostic outcomes. For Diagnostic Assessments that include Support and/or Treatment planning, additional fees apply for record review and report writing, which are not billed to insurance (see Fees & Payment Information section below).

OPWDD Evaluation


OPWDD evaluation services are appropriate for individuals, ages 12 through adulthood, who
  • Are applying for the first time to OPWDD and require confirmation of their autism spectrum disorder (ASD) and current adaptive testing.
  • Have had their application denied by OPWDD and require confirmation of a previously diagnosed ASD and current adaptive testing.
  • Have had their application denied by OPWDD because they are over 22 years of age and have never been diagnosed with ASD.
  • Have been referred by OPWDD or another agency for a diagnostic assessment to determine/confirm if they have ASD.
OPWDD assessment includes a review of records, interview and direct assessment with the patient, interview with one or more caregivers, completion of rating scales by the patient (if an adult) and by one or more caregivers, and a report that summarizes assessment findings, and provides a diagnosis and recommendations.

Guardianship Evaluation


Guardianship Assessment services are appropriate for individuals who require a Licensed Psychologist to evaluate an adult with autism spectrum disorder’s ability to make decisions for him/herself.

Guardianship evaluation requires a review of records documenting the individual’s ASD and his/her developmental levels, an interview of the person with ASD for direct evaluation of his/her ability to understand his/her rights for personal decision-making and the relinquishing of those rights to a guardian, and the preparation and notarization of the Affidavit of Examining Licensed Psychologist by a NYS certified notary.

Workplace Support Assessment


Workplace support assessment services are appropriate for individuals with autism spectrum disorder, 21 years of age and older, who are employed but who do not qualify for or receive vocational assistance through NYS or another agency.

Workplace support assessment includes interview for workplace description and job responsibilities, review of workplace policies and procedures, the completion of questionnaires to identify areas of support need, the development of support recommendations, and a report summarizing the individual’s support needs and the support recommendations.

Fees & Payment Information


Diagnostic Assessment:
  • Typically requires 3 office visits to complete.
  • Office visits can be billed to insurance. Self-Pay is also accepted (full fees apply - see office visit fees below).
  • Additional Services requested by you at the time of Intake (example: support and/or treatment planning require additional clinical work such as additional record review, use of additional assessment measures/methods and concomitant data analysis and report writing). These services are NOT billed to insurance (full fees apply - see office visit fees below).
  • Insurances Accepted: Excellus BCBS (Blue Choice Option, Essential Plan, HMO Blue Option, Blue Option Plus, Child Health Plus and Medicaid are NOT accepted), Aetna, and Cigna.
  • Methods of Payment Accepted: Cash, check, and credit card or HSA card.
Office Visit Fees:
  • 1st & 2nd Visits: $200 each (deductible/co-pay applies if billed to insurance)
  • 3rd Visit: $150 (deductible/co-pay applies if billed to insurance)
  • Additional Service Fees: $150 per hour / $75 per 30-minutes of work performed (estimated hours discussed with you prior to the work being performed)
OPWDD Evaluation:
  • Typically requires 4 office visits to complete.
  • Flat Fee: $2,000
  • Methods of Payment Accepted: Cash, check, and credit card.
Guardianship Evaluation:
  • Requires 1 office visit to complete the competency interview.
  • Flat Fee: $375
  • Methods of Payment Accepted: Cash, check, and credit card.
Workplace Support Assessment:
  • Typically requires 3-4 office visits.
  • Flat Fee: $1,000
  • Methods of Payment Accepted: Cash, check, and credit card.
